The Cade Foundation Illinois Race for the Family, hosted by the Fertility Centers of Illinois,  is a 5k run/walk, a 1 mile Family Fun Walk and  Savannah's Playground, a free outside festival for children and their parents/caregivers. Race for the Family is a heart warming event that that raises money to help infertile couples in become loving parents! This year is the  first year for the Cade Foundation Illinois Race for the Family and Savannah’s Playground. Our goal is to raise $50,000 for this years event.  Sign up today and help support families OVERCOMING infertility!

Please note that t-shirts, packets, and bags will not be available after the race.  Please obtain items at packet pickup or the day of the race.  Also, shirts are not guaranteed to anyone who regsiters after June 19, 2012.

IL Race for the Family Waiver and Release Of Claims, Assumption Of Risk
and Warning Of Risk
I understand that participating in a 5K run/walk or fun run is a
potentially hazardous activity, and I agree not to enter and participate
unless I am medically able.  I agree to abide by any decision of a race
official relative to my ability to safely complete the race and to obey
civil and criminal laws at all times, including traffic and pedestrian
laws.  I assume all risks from participating in this event and its related
activities including, but not limited to: falls; contact with other
participants; effects of the weather, including high heat,
humidity, precipitation, and the conditions of the road with all such risks
being known and appreciated by me.  I understand that participants are
expected to exhibit appropriate behavior at all times, including respect
for all people, equipment, and facilities and the race organizers may
dismiss anyone whose behavior endangers safety or negatively affects a
race, a person, a facility, or property of any type or kind.  I understand
that all entries are final with no refunds, the race director reserves the
right in the event of an emergency or local or national disaster to cancel
the race or to change the day and/or time to a later day and in the event
of cancellation or change there is no refund of entry fees.

Applications for minors will be accepted only with a parent's signature and
all children participating in the 5K race, Fun Walk or Savannah’s
Playground under the age of 15 must be accompanied by an adult during the
entire event.

I grant permission to the race organizers for the use of any photographs,
motion pictures, recordings, or any other record of this event for any
lawful purpose, including but not limited to promoting, advertising and
marketing purposes.  Any and all photographs, motion pictures, recordings
or other records of the event are and will become the sole property of the
Tinina Q Cade Foundation.  ****When registering online, I understand that
my online signature shall substitute for and have the same legal effect as
an original form signature.

IL Race for the Family is committed to conducting its race and activities
in a safe manner and holds the safety of participants in high regard. IL
Race for the Family continually strives to reduce such risks and insists
that all participants follow safety rules and instructions that are
designed to protect the participants’ safety. However, participants and
parents/guardians of minors registering for the race, programs/activities
must recognize that there is an inherent risk of injury when choosing to
participate in recreational activities/programs. You are solely responsible
for determining if you or your minor child/ward is physically fit and/or
skilled for the race or activities contemplated by this agreement. It is
always advisable, especially if the participant is pregnant or disabled in
any way or recently suffered an illness, injury or impairment, to consult a
physician before undertaking any physical activity.

Having read this waiver and release and knowing these facts and in
consideration of your accepting my entry fee and participation, I, for
myself and anyone entitled to act on my behalf, waive and release Tinina Q
Cade Foundation, Fertility Center of Illinois, race officials, volunteers,
sponsors and their respective affiliates, subsidiaries, principals,
directors, agents, officers, employees or contractors (hereafter the “IL
Race for the Family”) of and from all claims, liabilities or causes of
action of any kind arising out of my participation in this event or its
related activities, which I or my successors, assigns or heirs may ever
have now or in the future against any of them.  I further agree to waive
and relinquish all such claims my minor child/ward may have (or that accrue
to me or my child/ward) as a result of participating in these activities
against the IL Race for the Family. This release and waiver extends to all
claims of every kind and nature whatsoever.
